  No. We will collect the email addresses so that you can reach out to customers who fill out the form.

• Do users have to download any software to fill the forms on my site?

  No. HostMyPDF is a web-based service. No download or installation is needed.

• How long are filled copies of the form kept on my account?

  We will keep the filled out forms accessible through your history page, for 60 days after they are submitted.

• What parts of the form-filling interface can I personalize and localize?

  You can remove any non-essential elements of our interface, such as the ability to add a signature or picture, checkmarks, or font sizing options. You can also add your logo as well as default fonts, sizes, and colors.

MyForm is an IPad/Iphone module for the users of HostMyPDF service. HostMyPDF allows publishers to add fillable pdf forms to their web sites and grants access to these forms to their clients, vendors, and employees. HostMypdf is typically used by Medical offices, Insurance agents, Educational institutions and other businesses in order to capture customer information, streamline data entry, and other form related tasks.

MyForm allows the end users to fill and sign the published forms that are hosted on HostMyPdf. Every form is identified by a unique code.

Once the code is entered, MyForm lets the end user fill the pdf form, insert pictures and sign the form. The form is then delivered to the publisher and unless otherwise specified, all the confidential data is erased from the End user’s device.

It is our policy to respect the privacy of our publishers and users regarding any information that we may collect while operating our website and our mobile products. • Payment details: Publisher’s payments are processed directly through paypal or through a credit card processing company. We do not have access to and do not store publishers credit card or paypal payment information.

• Personal Information You Provide to Us: The types of Personal Information collected may include publishers username, email address, name of the file published, industry, information regarding publisher’s use of our Service, and browser/mobile device information. The Personal Information a publisher provides is used to allow the publisher: to set up an account and profile, to securely access their HostMyPdf account, to track the forms that they have published, and to deliver the forms they have published to an email address that they have preselected. The Personal Information could also be used for improving the content of the Service, customizing the advertising and content the publishers and users see, and communicating with the publishers and users about specials and new features.

• Information Collected Automatically: When you use the Service, HostMyPdf automatically receives and records information on our server logs from the publisher’s or user’s browser or mobile platform, including their IP address, the time of their visit, and cookie information. We treat this data as non-Personal Information, except where we are required to do otherwise under applicable law. HostMyPdf only uses this data in aggregate form. We may provide aggregate information to our partners about how our customers, collectively, use our site, so that our partners may also understand how often people use their services and our Service.

• User’s Personal Information contained in filled out published pdf forms: Published Forms filled out by users are stored on the server. However, these forms and the users information contained therein are accessible only to the publisher. The data contained in these forms is encrypted and is unreadable and unusable for anyone else including HostMyPdf. Publisher has the option to remove a filled out form at any point or wait for it to be deleted automatically within 30 days.

• Information Collected Automatically: When you use the Service, HostMyPdf automatically receives and records information on our server logs from the publisher’s or user’s browser or mobile platform, including their IP address, the time of their visit, and cookie information.
